“At this time, we are exclusively accepting PA Health and Wellness insurance. If you are a new patient, please ensure you select PA Health and Wellness as your Managed Care Organization (MCO) when visited by County Assistance Office. This will allow you to continue receiving care with us.”

STEP 1: Contact PA Independent Enrollment Broker (PA IEB) and Schedule Your First Intake Visit.

  1. Call PA IEB at 1-877-550-4227 to inquire about Home and Community Based Waiver Services. If applying online visit https://paieb.com/en/waiver-application

  2. Inform PA IEB of your intention to apply for waiver services and schedule your first in-home visit. For online applicant, It is a step 2 in online application called “Schedule a Visit (IVA – Intake Visit Assessment)”.

  3. PA IEB sends a Physician’s Certification Form to your doctor, who needs to fill it out and send it back to PA IEB.

  4. Timeline: Generally, 2 weeks to 4 weeks, depending on your doctor’s response and PA IEB availability.

STEP 2: Schedule and Have Your First In-Home Visit

  1. During the visit, discuss your needs and abilities with a PA IEB representative to determine if waiver services are suitable.

  2. Complete the Medical Assistance application (PA 600) and other required forms.

  3. Ensure all forms are sent to the County Assistance Office (CAO) to avoid delays.

  4. Timeline: Usually within 14 to 30 days.

STEP 3: Schedule and Complete Your Second In-Home Assessment Visit

  1. After the first in-home visit, PA IEB requests your local Area Agency on Aging (AAA) to conduct a Functional Eligibility Determination.

  2. AAA schedules your second in-home visit to assess your eligibility for waiver services.

  3. AAA sends the results to PA IEB for review.

  4. Timeline: Usually within 14 to 30 days.

STEP 4: PA IEB Reviews Your Physician’s Certification Form and Functional Eligibility Determination

  1. PA IEB needs the Physician’s Certification Form and Functional Eligibility Determination within 86 days of the first in-home visit.

  2. If the forms are not received, PA IEB contacts your doctor and you.

  3. Upon receiving the forms, PA IEB reviews them. If eligible, your application is sent to the CAO.

  4. Timeline: Usually within 14 to 45 days.

STEP 5: CAO Reviews Your Waiver Application and Makes a Decision

  1. CAO reviews your waiver application for financial eligibility.

  2. If anything is missing, CAO informs you how to provide it.

  3. Once complete, CAO approves or denies your application.

  4. CAO sends notices to you, PA IEB, and your chosen Managed Care Organization/Service Coordination Agency.

  5. You receive a decision within 90 days of PA IEB starting your waiver application.

  6. If no decision within 90 days, you can request a fair hearing.

Outcome:

  1. If approved, congratulations! You are enrolled in Home and Community Based Waiver Services.

  2. If denied, you can request an appeal hearing if you disagree with the decision.
