Onboarding Caregiver Patient Checklist

Dear Caregiver and Patient, Before your upcoming in-home visit, please gather the following items for a smooth onboarding process. This is just a checklist.

For Caregiver:

Photo ID [Driver’s License / State ID / Passport].

Proof of Two Year Old PA Residency (two year old utility bill, expired Driver’s License, two year old lease).

Social Security Physical Card.

Void Bank Check for Direct Deposit.

For Patient:

Medicaid Health Insurance Card.

Social Security Card.

Photo ID.

Emergency Contact Name and Number.
